SUBJECT: Evaluation Form for Student Teaching


EKU's evaluation form is designed to provide a profile of the pre-service teacher candidate’s teaching performance. This form should be utilized as a continuous assessment of the student teacher's ability to perform in a classroom environment. The evaluation form should help you to identify the effectiveness of the candidate and assess his/her potential as a teacher.


Competencies essential to the student teacher's profile are included in the attached evaluation. Careful analysis of each competency, indicating a the candidate’s strengths and weaknesses will provide you and the University supervisor an opportunity to record as SATISFACTORY (S) or UNSATISFACTORY (U) grade to the candidate's teaching performance.


The format lists the competencies to be evaluated on the top of each page, followed by a rating scale for each competency and an area below for your comments on each page. You will note the overall rating of your student teacher and her / his potential to succeed as a teacher listed on page 7. It is quite conceivable that a student teacher could be rated outstanding and receive all “excellent” checkmarks on the evaluation.  Do not evaluate the pre-service teacher in comparison with veteran teachers, but to what degree this individual meets the expectations for a beginning teacher. Please return the completed form to the student's University supervisor at EKU. Thank you!

Descriptors for evaluating the items in this evaluation:
EXCELLENT
Demonstrates excellence in all areas of the New Teacher Standards; ranks among the best student teachers (5%) with whom I have had the opportunity to work.
ABOVE AVERAGE
Demonstrates the potential to become an excellent teacher based on the Kentucky New Teacher Standards; one of the better student teachers with whom I have worked.
AVERAGE
Demonstrates the potential to become an effective teacher if placed under a master teacher during the initial year of teaching. This student teacher needs to demonstrate further growth in the areas indicated.
BELOW AVERAGE
Experienced considerable difficulties during student teaching; barely meets the minimum requirements for the Kentucky New Teacher Standards. This student teacher must be placed under the supervision of a master teacher during the non-tenure phase of his/her teaching career and demonstrate considerable growth in order to become an effective teacher.
UNSATISFACTORY
Does not meet the Kentucky New Teacher Standards and will need to discuss further options with the Director of Professional Laboratory Experiences
